EHS 0745 Hepatitis B Vaccination Questionnaire

You have been assigned EHS 0745 Hepatitis B Medical Surveillance requirement because you belong to a Work Planning and Control activity indicating you may be exposed to materials that may contain the hepatitis B virus (HBV).

How to complete your medical surveillance requirement:
You will receive credit for EHS 0745 after your completed BBP questionnaire is reviewed by Health Services

You must indicate one of the following:

• You wish to be vaccinated and receive the vaccination series, or
• You wish to decline the vaccination at this time (you may be vaccinated in the future if you decide)
• You have been previously vaccinated for hepatitis B

What to do:

• Go to clinic.lbl.gov to access the OHR (Occupational Health Records) Employee Portal 
• Click on “Message Center” link at the Home page
• Click on the “begin” button for the BBP Questionnaire

If you have questions about how to participate in the health screening, please review the medical surveillance information in your WPC Activity, discuss with your Activity Lead or Division Safety Coordinator, or the Medical Surveillance Program Manager, or contact Health Services at x6266.  If you feel that this enrollment is in error, please discuss with your Activity Lead or the Biosafety Subject Matter Expert.
