Registering for EHS0288 - Baseline Laser Eye Exam:

Description: All Class 3B and Class 4 laser users who will be at Berkeley Lab for more than 60 days must complete a baseline laser eye examination.

BEFORE you register for a laser eye exam, please make sure the WPC Activity that triggered the exam requirement is active and you are authorized to work by your supervisor under that WPC Activity. If you are not authorized or if the WPC Activity is not active, you will not be able to register for the exam at Health Services. After you're authorized in the WPC Activity, wait one day before you schedule the exam.

To register for a laser eye exam, go to clinic.lbl.gov, under "Message Center", complete the "Laser Vision-Surveillance Evaluation Acceptance" and schedule an appointment with the optometrist.If you have questions about the laser eye exam requirement, please discuss with your Activity Lead, supervisor, Division Safety Coordinator, or the Laser Safety Officer or the Medical Surveillance Program Manager.
